If the safe environment procedures for youth are not followed this form must be filled out and given to the safe environment director
YOUTH CODE OF CONDUCT NONCOMPLIANCE FORM

As part of the safe environment process every volunteer and staff member must be interviewed using this form which should be given to the safe environment director.
SAFE ENVIRONMENT INTERVIEW FORM (to print)

This form is required by the company that does the background investigation.  Download the form and fill it out.  Give or send the filled out form to the safe environment director who will then request the background.
DRIVER BACKGROUND AUTHORIZATION FORM

This form is required by the Diocese of Dallas.  Download it and fill it out. The safe environment director needs this form along with a copy of your drivers license and insurance information in order to complete the background process. Insurance must include the following coverage: bodily injury - $100,000 each person/$300,000 each accident; property damage - $25,000 and uninsured motorist.
